Insurance Claims and Clinical Histories

Most pet insurance claims do not require you to request your pet's clinical history from us.

In most cases, your insurance company will contact Boundary Road Veterinary Hospital directly if they require your pet's medical records or additional clinical information to assess your claim.

Some insurers, however, ask policy holders to provide the clinical history themselves or to arrange for the records to be supplied. If this happens, we're happy to assist.

The easiest option – Ask your insurer to contact us

To help protect your privacy and comply with the recommendations of our professional indemnity insurer, Boundary Road Veterinary Hospital does not provide patient clinical histories directly to clients.

Instead, we are happy to provide clinical histories directly to insurance companies upon request.

To make this easier, we have provided a downloadable letter that you can upload with your online insurance claim \. The letter simply advises your insurer that they can request your pet's clinical history directly from us.

For many insurance claims, this is all that is required and you will not need to complete the request form below.

If your insurer has specifically asked you to request the records

If your insurance company has instructed you to arrange your pet's clinical history, please complete the request form below.

Once we receive your request, our team will verify the information provided and send the clinical history directly to your insurance company.

Please note that we do not send patient clinical histories to clients.

What you'll need

Having the following information available will help us process your request more efficiently:

  • Owner's name as recorded with our hospital

  • Your contact details

  • Your pet's name

  • Insurance company

  • Insurance policy number (if available)

  • Insurance company email address (if known)

  • A copy of any request from your insurer (optional)

Processing Times

Clinical history requests are processed by our team during normal business hours.

While we endeavour to complete requests promptly, please allow a reasonable amount of time for processing, particularly during busy periods or where additional verification is required.

If your insurer contacts us directly after receiving the downloadable authority letter, they can simply email their request to us and we will respond directly.

Before completing this form

If your insurer accepts requests directly from veterinary hospitals, you may simply upload our authority letter instead.
